STEP 1: MEDICAL HISTORY
To get started either fill out the safe and secure e-Sign Medical History Form or download the form and email to stavan@iaamed.com. If you opt to, you can print out the form. Scan and e-mail it to: stavan@iaamed.com or fax it to (502) 451-7737.
STEP 2: LAB TESTING
Lab Orders will be sent/mailed to you for the closest to LabCorp to your home/work. We will provide all directions, steps and the actual order for lab techs. These are fasting so drink lots of water the ay before and day of the labs.
STEP 3: APPOINTMENT
Following Completion of all lab work and compilation of your “Health Baseline” you will be scheduled for an appointment with our healthcare professionals on staff for a 90-120 min one on one consultation and exam.
